Custom Care Planning

Too many times we’ve seen the emotional trauma of an unexpected and catastrophic event send families into places where they make very emotional decisions. Families get stuck fighting amongst themselves to decide on what care is best for mom or dad. In this emotional battle, sometimes what mom or dad wanted inadvertently gets pushed to the side. The reality is that so many people don’t think about who will care for them in the event of a sudden injury or disease. The sad and preventable reason this happens is because a plan was not even created or detailed out in black and white. These types of life events can quickly become some of the most detrimental, depressive or disruptive events in any families’ journey, but they don’t have to.

Our In Home Assessment Sessions are designed to help you create and communicate your plan. Our experienced Care Managers guide and support you every step of the way.

  • Discuss your current passions and quality of life.
  • Discuss your living situation and family situation.
  • Discuss family history of medical conditions, any current medical conditions and the possibility of care needs as life progresses.
  • Provide you with information and education on different types of care, such as home care, independent living communities, assisted living communities, memory care neighborhoods, board and care facilities and more.
  • Review the costs for the various types of care.
